Participation and emergency care

Wrestling is a contact sport. Training and competition can cause injuries, including serious injuries. I understand these risks and give permission for the athlete named above to participate in CWC Elite practices if approved. I will provide accurate health and emergency information and keep it current. If I cannot be reached during an emergency, I authorize CWC staff to seek appropriate emergency care for the athlete. A separate registration waiver must also be completed before participation.

Conduct and coach direction

Horseplay and lack of focus are not tolerated. A coach may direct an athlete to step out for the evening when the coach believes it is best for safety, conduct, or the athlete’s need for a mental break. The athlete and responsible adult agree to follow that direction.
